Medication management is an outpatient treatment that evaluates the need for psychotropic medications. MMS involves the initial evaluation, medication education to explain possible side effects, safety, and effectiveness, and ongoing medical monitoring related to the patient’s use of the psychotropic medication by a qualified healthcare provider. 

 

• We take a patient-centered approach to care. The service is individualized for each specific patient and focuses on the patient’s needs and concerns by involving the patient in the care process. 

• We will assess medication appropriateness, safety, and adherence, and consideration is given to the accessibility and cost of the medications. 

• We employ a collaborative approach to care that involves the patient, caregiver(s), pharmacists, and other healthcare providers.
